Construit en 1875 à New York dans les profondeurs d'une ancienne mine de grès, l'hôpital Brinkvale renferme les criminels les plus extrêmes : trop dangereux pour l'asile, trop déséquilibrés pour la prison. C'est là que Zachary Talylor, thérapeute, doit analyser la personnalité de Martin Grace, afin de déterminer si celui-ci est suffisamment sain d'esprit pour répondre pénalement des crimes dont on l'accuse.
